Our clinical model starts by collaborating with a plan to acquire members that are eligible and at high risk of poor outcomes.  We work together to sign members up to the Summer Health platform and build trust quickly by providing quality and easy-to-access care.

We provide virtual care (urgent and non-urgent), collect medical history, and share diagnostic kits so that members can easily engage from home.  The member is assigned a provider that coordinates their care and can build a relationship with the member’s local POC and other resources.
